Program Exclusions and Limitations
The program is a savings plan, not a dental insurance plan, and is secondary to any other dental plan. It cannot be used:
- In conjunction with another dental plan
- For services for injuries covered under workman’s compensation
- For referrals to specialists
- For hospitalizations or hospital charges of any kind
- For costs of dental care which is covered under automobile medical
- In conjunction with coupons or cash discounts
- For all products and cosmetic treatments (bleaching kits, toothbrushes, BOTOX, ULTHERAPY)
When Care Credit is used in conjunction with one of our affordable dental plans, your member discount is 5%.
Your dental savings plan will not apply to dental care for which, in the sole opinion of the treating dentist or doctor, lies outside the realm of their capacity.
The dental savings plans outlined above are only honored at Lacamas Dental.
The dental assistance savings plan is not an insurance plan that can be used with another dental office.
Program Guidelines
- No other discounts with this offer
- Membership is not for re-sale
- Membership valid for one year after issuance
- Patient’s portion of bill due at time of service
- NON-REFUNDABLE. No refunds of premiums will be issued at any time if participant decides not to utilize dental plan
- Savings plan duration is for one full year, starting on the registration date and expiring at the end of the membership year
